In order to ensure the safety of your child(ren) while in our care, we require your consent for the following: *

I give permission for my child(ren) to take part in the normal activities of this department, including the use of the GP Kids Playground and all equipment within this space.

In the event of illness or accident, I give permission for appropriate persons to administer first aid or medical treatment as required.

In the event that my child(ren) requires emergency hospital treatment, we will make every effort to contact you immediately. However, if we are unable to reach you, I give permission for an adult leader to sign consent on my behalf.

Data Protection (GDPR)
Your personal data will be treated as strictly confidential and will only be shared with relevant Staff and Ministry/Lifegroup Leaders of the Church in order to carry out pastoral care or for purposes connected with the Church. This includes sharing your data with our partner charities Connect Ministries and Compassion Ministries where relevant. We will not normally share your personal data with any third party apart from appropriate external regulators and authorities (such as HMRC , HSE etc) and will only share your data with other third parties with your consent.

Permission for photographs or videos of my child(ren) to be taken for church use, including church social media platforms.
